Maybe the guy in question is a gynecologist and he is either freezing off her genital warts or plucking crabs off one by one.
Maybe another guy could be a tattoo artist. The woman was born with a genetic deficit which prevents her from growing pubic hair and she has decided to get a "Hair Tattoo" image made in the land down below.
Maybe she is having a plaster cast transfer made from her secret garden for the sex toy industry.

It is an ego thing. I know a woman in our town who loves to do that. She thinks she is a hot little number. I heard her comment one time "I'm sitting on a gold mine" in reference to her vagina.
She thinks that fanning open her crotch makes men crazy with desire and that, if they look, it means that they want her. I asked her about why she does it one time and she said she likes to "torture them" by being able to look but not do anything about it.
Unbeknowist to her, when they look it is like when people look at a train wreck. She's not turning anyone on by behaving so disgustingly (IMHO).

I remember playing some fast pitch softball and one gal would always come and watch us play, she always flashed us, The crotch shots were quite impressive made it hard to concentrate on softball..
