Im not old yet, and my sack is still very firm.
I will have to say that yes it does though. My buddy is an administrator at a nursing home, and he told me that he had to remind the nurses not to set this old man down on his ballsack when they put him in his wheelchair. I also think that as your nuts arent being drained of semen regularly that they become heavier.......Im not sure though cuz I dont let mine fill up.

In an answer... The cremasteric muscle weakens and although no more skin is grown, the scrota tend to always descind; unless extreme cold is encountered. This appears to be as the titled term, to sag.
...Testes enlarge up to about the age of 40 to 50 and thus, the scrota also enlarges to encompass. Up or down depends on the regulatory of this cremasteric muscle.
...Heat and hot weather descind and cold rescinds or retracts through the actions of this muscle. Cold or heat extremes kill sperm production and thus, nature's way of protecting and keeping the testicles comfortable and productive.
...If you men are reading this, the prostrate is our weakest muscle. Much like a woman practices kegels, we need to strengthen our prostrate. Obviously, this helps in old age and urine flow and control... but during orgasm, a strong prostrate muscle will stop the flow of semen and allow male multiple orgasms.
