I personally think that if it's something your lover really likes and you know that you are really turning him on that it would turn you on as well. Try role playing be the naughty nurse and wear a rubber glove and use some lube. Playing doctor in this instance could help you with the reluctance of getting any stuff under your nail and might just improve the situation.
